QUESTIONNAIRE FOR JEWISH PHYSICIAN
Four page questionnaire for Ancel Szkolnikow, from Warsaw, a Polish Jew who worked in the medical field, contains his biographical information including his religion, which he declares a jewish faith, letter-sized, brittle edges. After 1939 Jewish medical personnel, including physicians, dentists, nurses, medical orderlies, midwives, and those in other health-related occupations, were required by the administrative authorities of the German occupation to complete a questionnaire giving personal information. This information served the Nazis when they sorted the Jews in the ghettos.
