Signed by Rabbi David Tuvya of Łomża, Rabbi Fischel Levi [Rubin] and Rabbi Mordechai Meir Robinson.

חתומים על המסמך:
רבי דוד טביא מלומזא (תקס"ג-תרכ"ג), ממייסדי וראשי כולל פולין. אבי משפחת ילין הירושלמית. כשעלה ארצה החליט להחליף את שם משפחתו לעברי. "יעלין" בפולנית פירושו "צבי", ומאחר ששם זה בתור שם פרטי אינו מתאים לשם-משפחה בחר בתרגומו הארמי טביא וכך היה חותם: דוד טביא מלאמזא , ויהודי ירושלים היו קוראים לו על שם מקום מוצאו: ר' דוד לומזר.
רבי פישל לוי [רובין] ממונה הכולל.
רבי מרדכי מאיר רובינזון – אב"ד מזריטש בפולין, ולאחר עלותו ארצה כיהן כדיין בירושלים. נסתלק בשנת תרכ"א.
