Minhag Avos Tanya – minhagim and halachos by Rabbi Yechiel ben Yekusiel Anav. Matnua [1514], at the press of Shmuel Latif. First edition.
The sefer is organized as a halachic work that accompanies the siddur per the Italian minhag, a summary adaptation of the Shivlei Leket, with many additions.
Printed without the author's name. Over time, researchers tried to identify him and, today, most believe that it was written by Rabbi Yechiel ben Yekusiel br' Binyamin HaRofei.
Since it was printed without a title page, its generally called by its first words.
The colophon states: Finished Minhag Avos sefer HaTanya in the month of Sivan in the year 5000 [and 200] and 74 since Creation.]
In 1800, it was printed with the title Tanya Rabasi, and that's how it's been known ever since. (Perhaps to differentiate between the Likutei Amarim-Tanya that was printed a few years earlier.)

[1,99] leaves. The first leaf: It's hard to decide if this is an original leaf or reproduction, followed by 4 leaves that have been supplied from a different copy These leaves are blemished, with loss, but have been professionally filled in with paper and reproductions. The rest of the sefer is in overall fine condition, other than a few aging stains.
